Clinical Pharmacy Specialist

The Clinical Pharmacy Specialist will provide comprehensive medication management and reconciliation, partnering with physicians and care teams to create CPA-based care plans. The role involves monitoring therapy effectiveness and safety, intervening as necessary, and supporting quality improvement and REMS initiatives.

Key Responsibilities

• Provide comprehensive medication management and reconciliation.
• Partner with physicians and care teams to order CPA-based care plans.
• Monitor therapy effectiveness and safety; intervene as needed.
• Support quality improvement and REMS initiatives.
• Assist with complex prior authorizations and workflow improvements.
• Educate patients and support ongoing clinical development.

Required Skills & Qualifications

• Doctor of Pharmacy (PharmD).
• Licensed pharmacist in California.
• PGY2 Ambulatory Care Residency, or PGY1 + 1 year ambulatory under CPA, or 3+ years ambulatory practice with disease state management.
• Strong communication and organization skills.
• Proficient in technology and workflow management.
• Demonstrates empathy, integrity, and a team-first mindset.
