Ambulatory Care Pharmacist

The Ambulatory Care Pharmacist will be responsible for providing comprehensive medication management, ensuring optimal therapeutic outcomes for patients through collaboration with healthcare teams.

Key Responsibilities

• Conduct medication reconciliation and dose optimization.
• Promote medication adherence and perform drug-level assessments.
• Collaborate with physicians to develop individualized care plans.
• Monitor therapy effectiveness and safety, identifying optimization opportunities.
• Support operations by facilitating prior authorizations and patient education.
• Engage in ongoing clinical development and streamline workflows.

Required Skills & Qualifications

• PharmD degree and CA pharmacist license.
• Board certification (e.g., BCPS).
• Completion of PGY2 residency or equivalent direct patient care experience.
• 3+ years of clinical pharmacist experience.
• Strong communication and organizational skills.
• Proficient in technology relevant to pharmacy practice.
