The Case Manager, RN is responsible for conducting nursing assessments in a clinic setting and participating in an Interdisciplinary Team (IDT) to develop and revise care plans for patients. This role involves delivering and documenting nursing interventions according to the established care plans, administering medications, and collaborating closely with the PACE Physician and the IDT, including engaging with families to instruct them on disease management, self-care, and prevention strategies.

Key Responsibilities
  • Conduct clinic nursing assessments.
  • Participate in IDT to develop and revise care plans.
  • Deliver and document nursing interventions per care plans.
  • Administer medications as prescribed.
  • Collaborate with PACE Physician and IDT.
  • Instruct patients and families on disease management and self-care.
Required Skills & Qualifications
  • Graduate of an accredited nursing program; BSN preferred.
  • Unencumbered California RN license.
  • Current BLS/First Aid certification.
  • Two years nursing experience with frail/elderly population preferred.
  • Must obtain CPR and First Aid within 180 days of hire; maintain certification.
