As a Senior Living Home Care RN Manager, you will lead and inspire a dedicated team to provide exceptional care to clients. You will collaborate closely with the Rehab Manager, Director, and DCO for effective oversight of field staff while ensuring high-quality clinical services.

Key Responsibilities
  • Lead, inspire, and motivate team members to deliver exceptional care.
  • Collaborate with management for oversight of field staff.
  • Review clinical documentation for accuracy and quality.
  • Assign clinicians to clients in coordination with the office team.
  • Develop and update client care plans based on assessments.
  • Coordinate client care and referrals, acting as an internal case manager.
  • Participate in hiring and interviewing field staff as needed.
  • Represent the organization in community and physician visits.
Requirements & Qualifications
  • Minimum of 2 years RN experience in Home Health.
  • At least 2 years in a leadership or supervisory role.
  • Excellent organizational and communication skills.
  • Strong desire to lead and motivate a team.
  • In-depth knowledge of home health policies and procedures.
