We are seeking a dedicated Physical Therapist to join our team, providing assessments and interventions in both facility and home settings. The ideal candidate will collaborate with an interdisciplinary team to develop personalized care plans for our participants.

Key Responsibilities
  • Conduct PT assessments to define care needs and goals.
  • Participate in interdisciplinary team meetings to formulate care plans.
  • Deliver and document interventions according to care plans.
  • Provide active and passive exercises, gait training, and assistance with activities of daily living (ADLs).
  • Supervise Therapy Assistants and guide the implementation of functional maintenance programs.
  • Train staff, participants, and caregivers on the use of therapy equipment.
Requirements & Qualifications
  • Bachelor’s degree in Physical Therapy; Master’s or Doctoral degree preferred.
  • Two years of clinical experience with a frail/elderly population preferred.
  • California Physical Therapy license or eligibility.
  • CPR and First Aid certification within 180 days of hire.
  • Maintain CPR/First Aid certification thereafter; hire preferred.
