The RN Nurse Manager for Home Care will be responsible for visiting geriatric clients in Auburndale, MA, and surrounding areas to conduct assessments and develop client care plans. This role involves coordination among clients, field staff, office personnel, and clinicians to ensure exceptional home health services are delivered.

Key Responsibilities

Visit geriatric clients and conduct assessments.
Develop and execute client care plans.
Coordinate among clients, field staff, office, and clinicians.
Supervise and support Home Health Aide staff in the field.
Facilitate new hire orientation for Home Health Aides/CNAs and participate in training.
Provide exceptional care and contribute to high-quality home health services.

Requirements & Qualifications

Registered Nurse (RN) with 2+ years of RN experience.
Home Care experience preferred.
Supervisory/managerial experience preferred.
Flexible hours; 10-25 hours/week.
