We are seeking a dedicated Occupational Therapist to conduct assessments and deliver tailored interventions for our participants in both facility and home settings. The ideal candidate will actively participate in interdisciplinary team meetings to develop comprehensive care plans and implement effective treatment strategies.

Key Responsibilities

Conduct OT assessments to define participant needs and goals.
Actively participate in interdisciplinary team (IDT) meetings to develop participant care plans.
Deliver and document interventions as per participant care plans.
Provide active and passive exercise, gait training, and activities of daily living (ADL) training.
Collaborate with Therapy Assistants to implement Functional Maintenance Programs (FMP).

Requirements & Qualifications

Bachelor’s degree in Occupational Therapy; Master’s preferred.
Two years of experience in a clinical setting with a frail/elderly population preferred.
Valid California Occupational Therapy License or license eligibility.
Must obtain CPR and First Aid certification within 180 days of hire; certification preferred.
