"Smooth Salesforce integration, but some learning curve"
I use FormAssembly to securely collect data and integrate it into Salesforce. I really like the prefill feature, which lets us input known data, saving time for form fillers. The Salesforce integration is key and very helpful because information goes straight into our system, streamlining the process. It's excellent, and I'd give it a 10 out of 10. However, there is a learning curve, which is expected but still noticeable. The page for building connectors loads slowly. Specifically, the page where we map form data to Salesforce fields using connectors can be sluggish. Also, the formula builder requires a specific syntax that takes time to learn. FormAssembly allows us to securely collect data, prefill forms, and integrate seamlessly with Salesforce, streamlining data entry and sending information directly into our system, making the process efficient.

"Versatile tool for all nonprofit needs"
I like that we can use FormAssembly for many aspects of our nonprofit's services. We use it for surveys, events, helping teachers book field trips, and collecting usage data from our Culture Pass libraries. I wish it had a built-in connector to Salesforce and other CRMs, which would make collating data into our database much easier. I also wish our old system wasn't being phased out. It's going to cost us a lot over the next few months to migrate to the new system because of how we have it connected to Salesforce via Zapier. It helps us capture data consistently in an easy-to-read and easy-to-use format.

The client services support and onboarding were absolutely outstanding. They are very mindful of the time they have with you. As a new user, they helped me plan next steps and figure out what to work on between sessions. At this point, I'd like a better way to generate reports on form submissions. That might be a gap in my knowledge. We're moving toward paperless forms and applications. For our applicants, particularly those with TBI and PTSD, the time saved and reduction in stress has been significant. Using connectors and workflows has reduced their need for face-to-face appointments with providers, which is a common challenge that often leads to avoidance and delays in submissions.

From an admin perspective, FormAssembly setup is fairly easy. The user interface is good (maybe not as simple as some other tools but more robust). The Salesforce connector, which our team uses most, is excellent and very easy to customize. FormAssembly also explicitly offers HIPAA-compliant tools, which is essential for us. Overall, it's an excellent tool in a space with few options. Because it's so robust and customizable, the learning curve is steeper than other survey tools. I wouldn't entrust a basic user with creating surveys that involve connections, as they'd likely run into problems quickly. We primarily use it to bring internal and external data into our CRM in a clean, customizable format.

"Powerful tool with improving support"
FormAssembly offers a lot of flexibility in a fairly simple development environment. We use forms for many purposes, including general data collection, higher education applications, enrollment, class registration, license renewals, etc. Most forms connect to Salesforce and our credit card processor (Authorize.net). The company seems committed to enhancements and product growth. Recently, we started using the iATS/Deluxe connector for ACH donations from US donors, and the HTTPS connector to pass data to Zapier for internal workflows. Support used to be just average, but response times have improved over the past few months. The documentation is a bit clunky to navigate, and some features or steps are undocumented. Some support reps know the product well and resolve issues quickly, while others struggle. So, support can be inconsistent. With FormAssembly, we can quickly create forms to replace fillable PDFs and eliminate data entry since forms connect to Salesforce.

What I appreciate most about FormAssembly is the flexibility and depth of its integrations with other tools, particularly Salesforce. The ability to create very specific connectors lets us design sophisticated workflows, like updating a particular Salesforce field based on a respondent's submission. Pre-fill connectors are especially useful, allowing respondents to see previously provided information, confirm it, and make updates if needed. This enhances the user experience and builds trust by showing we manage data carefully and accurately. I've also had consistently great experiences with FormAssembly's support team. They are responsive, knowledgeable, and helpful in resolving issues. FormAssembly keeps improving, and we've benefited from many new features. However, there are still some basic capabilities that would make form management and collaboration easier. I'd love to see the ability to export a form, add internal comments on individual fields, and have more collaboration tools during form development. I also wish administrators automatically had manager-level access to all forms, which would streamline workflows and troubleshooting in our environment. A model similar to Salesforce, where system admins can view and manage everything by default, would be helpful. Additionally, I'd like more reusable functionality for connectors. We can add questions and sections to predefined content, but I wish we could do the same with connector configurations. Creating and reusing standardized connector components would save time, reduce duplication, and ensure consistency across forms. FormAssembly is our main data collection platform, helping us gather clean, consistent, and usable data from the start. In-form validation catches common data quality issues before submission, and the Salesforce connectors let us control exactly how data is written back to our system. For my department, this significantly reduces manual data cleaning and troubleshooting after collection, giving us greater confidence in the data we use for program monitoring, evaluation, and reporting. This allows our team to focus more on generating insights and supporting decision-making rather than fixing data issues.

Form Assembly was a breeze to install, set up, and use. We connected to our Salesforce instance effortlessly and began building forms right away. The form builder is very user-friendly, and the options for question types, conditional visibility, and response types were exactly what we needed. There isn't much to complain about. The more branching logic we added, the more complex the interface became for form managers. It allows us to customize forms on our website to capture leads. We can run multiple forms across different pages and sites. Using FormAssembly, we can segment form submitters based on the URL they came from, their source, etc. The reporting feature helps us review submissions for completeness and determine which forms are most effective.

Form Assembly is very intuitive when it comes to building forms. You can preview exactly what your audience will see as you create it. The integration between forms and our software platform has greatly boosted efficiency across the whole organization and cut down on tedious manual data entry. The only downside I can think of is that learning to use the pre-fill and post-survey connectors can be tricky. However, there are plenty of templates to help, along with support tutorials and regular office hours from customer service. There's also an AI chatbot for error codes and general issues. Data entry used to consume a lot of time and effort organization-wide. Now, forms are uploaded instantly, staff get immediate notifications, and workflow efficiency has improved. Additionally, we previously used numerous spreadsheets and other tracking methods that are no longer needed thanks to Form Assembly.

My favorite aspect of FormAssembly is its versatility. Overall, FormAssembly has automated processes that once required multiple tools, spreadsheets, and manual hand-offs, giving us the flexibility to build solutions rapidly while still accommodating advanced integrations and conditional workflows as we grow. My biggest gripe is the challenge of getting timely, personalized support when a problem needs human intervention. While help articles and self-service resources are helpful, complex issues with forms, connectors, or workflows sometimes require direct assistance. Since phone support isn't available, live agents are hard to reach, and email replies can take days, slowing down urgent resolutions. A more dependable live chat option or quicker response times would greatly enhance the customer experience. Before FormAssembly, our processes relied on emails, spreadsheets, duplicate data entry, and manual follow-ups. Now, we can create custom forms and workflows that capture the right information, guide users based on their answers, and automatically send data to Salesforce or Google Sheets. We use it for customer-facing applications, internal request forms, surveys, event tracking, discount and financial aid processes, and operational data collection. The Salesforce integration has cut down on duplicate entry and made it easier to create or update records and cases without manual transfer. The Google Sheets integration gives other teams access to real-time data, reducing reporting delays and eliminating the need for frequent exports. FormAssembly has also let us build solutions in-house instead of relying on custom development for every new need, speeding up process launches, making updates easier, improving data accuracy, and saving substantial time on administrative tasks and manual hand-offs.

What stands out to me about FormAssembly is how adaptable it is and how well it links up with Salesforce. Building forms is straightforward, so updating them as our requirements shift is simple. However, handling extensive workflows can get messy, especially with numerous conditional rules.
