Social Worker Position

The Social Worker will conduct comprehensive psychosocial assessments to identify participants' needs, goals, and preferences. This role involves active participation in interdisciplinary team (IDT) meetings to develop effective care plans and the delivery and documentation of social work interventions including resource allocation, transitions, counseling, and case management.

Key Responsibilities

• Conduct psychosocial assessments to identify participant needs and goals.
• Participate in IDT meetings for care plan development.
• Deliver and document social work interventions effectively.
• Coordinate with Primary Care Physicians and IDT for smooth care transitions.
• Initiate and coordinate care conference meetings for team collaboration.
• Provide discharge planning for participants who disenroll from the program.

Requirements & Qualifications

• Master’s degree in social work (MSW) is required.
• A minimum of one year of experience with frail/elderly populations is preferred.
• Experience coordinating and facilitating care conference meetings is preferred.
• Experience assisting individuals with behavioral health or substance abuse issues is preferred.
• CPR and First Aid certification within 180 days of hire and maintained.
