Responsibilities
Perform in-person assessments with the interdisciplinary team to plan care
Coordinate discharge and placement for acute and long-term needs
Visit about 8 patients per day with home visits
Requirements & Qualifications
ACGME-accredited residency required
Master’s or Doctorate degree
Board Certification preferred
Active, unrestricted state medical license
CDS certification and DEA registration
CPR and First Aid certification within 180 days; hire-ready CPR/First Aid preferred
