Job Overview

As a Primary Care Advanced Practitioner, you will provide comprehensive primary care in outpatient settings, conduct routine visits, address acute concerns, perform annual wellness visits and risk assessments, educate patients on conditions, plans, and prevention, coordinate care with specialists and other providers, and use telemedicine for follow-up and consultations.

Requirements & Qualifications

• NP or PA with 2+ years in primary care
• Fluency in Mandarin or Cantonese
• Experience with Medicare patients and elder care
• Knowledge of ICD-10 coding and HCC risk adjustment
• Active, unrestricted license and DEA registration
• Medicare enrollment in good standing or eligibility
