Physical Therapist Position

The Physical Therapist will conduct assessments and provide therapy interventions in both facility and home settings. This role involves collaboration in interdisciplinary team meetings to develop personalized care plans for patients.

Key Responsibilities
  • Conduct physical therapy assessments in facility and home visits.
  • Participate in interdisciplinary team (IDT) meetings to develop care plans.
  • Deliver and document interventions as per care plans.
  • Provide active and passive exercise and gait training.
  • Supervise Therapy Assistants and guide therapy programs.
  • Train staff, participants, and caregivers on equipment use.
Requirements & Qualifications
  • Bachelor's degree in physical therapy; master's or doctoral degree preferred.
  • Minimum of 2 years clinical experience with frail elderly preferred.
  • California PT license or eligibility required.
  • CPR and First Aid certification required within 180 days of hire.
