Job Overview

Conduct OT assessments in facility and in-home. Participate in IDT meetings to develop care plans. Deliver and document interventions per care plans. Provide active and passive exercise, gait and ADL training. Work with Therapy Assistants to implement FMP.

Requirements & Qualifications

• Bachelor’s in OT; master’s preferred
• 2 years clinical experience with frail/elderly preferred
• Valid California OT license or license eligibility
• CPR/First Aid within 180 days; hire-cert preferred
• Outpatient/acute/inpatient or SNF experience preferred
