Hospice Registered Nurse

Provide home-based rehabilitative care and skilled nursing. Travel to clients in Gloucester, Salem, and Cumberland counties. Follow designated care plans and administer medications, wound care, and procedures. Monitor client conditions and report changes to the Clinical/Client Services Manager. Document observations and interventions using an EMR tablet. Collaborate with a multidisciplinary team to support patient needs. Valid NJ RN license. Minimum 1 year clinical (medical/surgical) experience. Home care experience preferred. Graduate from an accredited nursing program. Ability to work independently and manage time.
