Job Overview

Provide hands-on personal care and assistance with ADLs in participants' homes.

Responsibilities

• Assist with instrumental ADLs: laundry, light housekeeping, shopping, meals
• Measure and document participants' vital signs as instructed
• Act as a medical escort to appointments, supporting safe transfers
• Assist Rehabilitation team by carrying out home exercise programs per care plan
• Provide translation services as needed

Requirements & Qualifications

• High school diploma or equivalency
• Current BLS/First Aid Certification
• CNA or HHA certification preferred
• Bilingual preferred
• Must be registered with Home Care Registry prior to start date
• CPR and First Aid certification within 180 days of hire (preferred)
• Medical insurance coverage (Medical, Dental, Vision)
• 17 days PTO, 12 holidays observed, and 6 sick days
